Student Success Center Referrals

A new service is being offered by the Student Success Center to improve student retention and completion. If you have a student who is having difficulty succeeding in his/her class and classroom intervention has not solved the problem or if you have a student who is at high risk of having difficulty, you may use this form to refer the student to the Student Success Center. A SSC staff member will meet with your student to determine barriers that are affecting his/her progress and will work with the student to help them remove the barriers. A SSC staff member will contact you about the student’s plan and work with you to monitor the student’s progress and offer continued support.

You may also continue to refer students to the SSC for other services: tutoring, resource referral, study groups, learning style evaluations, guidance and mentoring, assistance with resume writing, email accounts and student identification cards
