Request for Graduates for 2008-2009

Dear Instructor/Dean/Advisor:

Please list prospective graduates from your program on the Request for Graduates for 2008-2009 Form. (Click on EGOS Staff Information Page at the bottom of the page. The form is under Staff Files, Graduation Forms).

You will need to include the following information:
· Student’s First Name
· Student’s Last Name
· Student ID#
· Program of Study
· Graduation Term
· Expected Date of Completion

The deadline for submission of your list is October 15, 2008. If you have student/s that begin class after October 15, 2008 and will complete their programs prior to January 1, 2009, submit this list, in the same format, as soon as possible, but not later than January 1, 2009. Graduation is January 30, 2009 at 10:00 am at the Colorado Convention Center. Rehearsal is at 8:00 am.

Please use the template that is on the Emily Griffith Staff Information Page. Please inform your students that they cannot pick up their certificates and transcripts until after March 1 - 15, 2009.
